For Physicians & Discharge Planners
Continuity of Care for Your Post-MVA Patients — At Home, Across Florida.
We close the follow-through gap between your clinic or hospital and the patient’s recovery. Licensed Florida DPTs evaluate and treat in the home, and you receive timely, structured communication on every case you refer.

The post-discharge follow-through gap.
Patients leave the ED, urgent care, or your office with a PT referral — and many never complete the intake. Pain, missing transportation, work conflicts, and care-coordination friction mean the plan you ordered doesn’t actually happen. The patient returns weeks later with worse outcomes that could have been prevented.
In-home continuity of care.
Licensed DPT in the home
Comprehensive evaluation and skilled treatment by a Florida-licensed Doctor of Physical Therapy.
Plan adherence
Removing transportation as a barrier produces measurably higher visit completion than outpatient referrals.
Closed-loop communication
You receive the initial evaluation, periodic progress notes, and the discharge summary — by secure fax or email.
Florida PIP referral pathway.
Under Florida’s Personal Injury Protection statute, a physician (MD, DO), dentist, chiropractor, or supervising ARNP/PA determination of an Emergency Medical Condition expands the patient’s available PIP benefit. We accept referrals from primary care, urgent care, orthopedics, physiatry, and hospital discharge planners, and we coordinate documentation requirements with the referring practice.
Statutory references provided for orientation. Coverage determinations and EMC documentation should be made by the qualified treating provider per current Florida law.
How to refer.
- 1. Send a referral via the secure form, by phone, or by fax.
- 2. Our intake team verifies insurance/PIP and schedules the in-home evaluation, typically within 48 hours.
- 3. You receive the evaluation report and ongoing communication through discharge.
