For Physicians & Discharge Planners

In-Home PT for Your Patients After a Crash — Anywhere in Florida.

We close the follow-through gap between your clinic or hospital and the patient’s recovery. Visits are delivered by a Florida-licensed physical therapist, or by a licensed PTA working under DPT supervision — and you receive timely, structured communication on every case you refer.

We specialize in auto-accident and personal-injury care. We bill the patient’s auto insurance for crash-related care. We do not bill regular health insurance or Medicare.

The post-discharge follow-through gap.

Patients leave the ED, urgent care, or your office with a PT referral — and many never complete the intake. Pain, missing transportation, work conflicts, and care-coordination friction mean the plan you ordered doesn’t actually happen. The patient returns weeks later with worse outcomes that could have been prevented.

What you get when you refer.

  • Licensed clinician in the home

Evaluation and skilled treatment by a Florida-licensed physical therapist, or a licensed PTA under DPT supervision.

  • Plan adherence

Removing transportation as a barrier produces measurably higher visit completion than outpatient referrals.

  • Reports back to you

Initial evaluation, periodic progress notes, and the discharge summary — sent by secure fax or email. Records requests turned around within two business days.

  • We tell you either way

If we cannot reach or schedule your patient, your office hears from us — no silent drop-offs. You always know the status of every referral.

Auto-accident referral pathway.

We accept referrals from primary care, urgent care, orthopedics, physiatry, and hospital discharge planners. Florida law requires the patient to be seen by a qualifying medical provider within 14 days of the crash — we coordinate the documentation flow with your practice so nothing falls through the cracks.

Coverage determinations and clinical documentation are made by the treating provider under Florida law — we handle the referral intake and follow-through.
