Physical Therapist (PT) Jobs in Palm Coast, FL — In-Home

Posted August 19, 2026 · PT Near Me · Contract / PRN / Part-time · Pay $60–100/hr

Palm Coast's planned residential grid and heavily retiree population make it a natural in-home PT market — post-joint-replacement recovery, gait and balance work, neuro cases, and crash injuries from I-95 and US-1. Outpatient options in Flagler County are limited, which pushes real clinical volume to home-based care.

From a Palm Coast base our PTs typically serve Bunnell, Flagler Beach, and north into Ormond Beach and Daytona Beach. The street grid is simple, addresses are easy to find, and visit-to-visit drives are usually under ten minutes.

Working as a PT in Palm Coast, Flagler County

Palm Coast sits in Flagler County. One of Florida's fastest-growing 65+ populations plus I-95 crash referrals keeps post-surgical, balance, and post-accident caseloads varied.

Quiet residential streets, easy parking, and short drives are the daily norm.

Communities you'd typically cover from a Palm Coast base

  • Bunnell
  • Hammock Beach
  • Ormond Beach
  • Palm Coast Plantation

Visits are always scoped to the coverage radius you set with the back-office — Palm Coast openings are never dispatched outside your stated service area.

Job description

PT Near Me is a Florida in-home physical therapy company. Our own licensed clinicians treat injury and post-accident patients one-on-one in their homes. We're hiring a licensed Physical Therapist (PT) to join our team — evaluate new patients in their home, author the plan of care, and lead progression and discharge. A strong fit for a PT who values clinical autonomy, flexible scheduling, and patient-centered care, with competitive per-visit pay, hours you set, and the freedom to add visits whenever you want. We handle scheduling and the administrative back-office so you can focus on treating.

What you'll do

  • Perform initial in-home evaluations for post-accident, post-surgical, and chronic-condition patients.
  • Author and update the plan of care — goals, frequency, interventions, and discharge criteria.
  • Deliver skilled treatment one-on-one in the patient's home — therapeutic exercise, manual therapy, neuromuscular re-education, gait and balance training.
  • Lead progression, modify the plan as patients respond, and coordinate discharge.
  • Direct and supervise PTA visits in accordance with Florida practice act requirements.
  • Document evaluations, daily notes, progress notes, and discharge summaries accurately.

What you need

  • Doctorate or Master's from an accredited Physical Therapy program.
  • Active Florida PT license (or license-eligible).
  • Strong clinical reasoning across orthopedic, neuromuscular, and post-trauma cases.
  • Reliable transportation and the ability to manage your time and service area independently.
  • Comfort owning the plan of care for in-home patients with limited equipment.
  • Home-based or outpatient orthopedic experience preferred.

What we offer

  • Competitive per-visit pay (evaluation and follow-up rates discussed by region during intake).
  • Flexible scheduling — set your own hours and caseload.
  • Minimal commute — patients in their homes, near you.
  • Scheduling and administrative back-office handled for you.
  • Supportive team and streamlined documentation tools.

Job type: Contract / PRN / Part-time. Specialty: Home Health / in-home rehab. Location: On the road, in patients' homes.

Florida-licensed PTs and PTAs.
